For 24-25 Three Catholic Churches Faith Formation Registration, please fill out the form below. After you register you will be directed to the payment page for online payment. Please remember to add on Sacrament Fees(2nd and 11th Grade) or Non-parishioner Fees.

All Grades $50 per child($150 cap per family)
Sacrament Fee (2nd and 11th Grade) Additional $50 per child
Non-parishioner Fee $50 per family
Early Bird Discount (August 1-5) —$5 per child
Late Fee (after August 25) +$5 per child
